START triage categorizes patients in mass casualty incidents: GREEN (minor/walking wounded), YELLOW (delayed, unable to walk but stable vital signs), RED (immediate, life-threatening but salvageable with rapid intervention), and BLACK (expectant/deceased, no breathing after repositioning airway). Assessment follows: Can they walk? (GREEN) → Are they breathing? (if no after airway opening → BLACK) → RR >30 or <10? (RED) → Radial pulse absent or cap refill >2 sec? (RED) → Can they follow commands? (if no → RED, if yes → YELLOW). In the prehospital setting, clinical decision-making follows a systematic approach beginning with scene safety and BSI precautions, followed by primary assessment (ABCs), and targeted interventions based on identified life threats. The EMT must prioritize interventions that address the most immediate threat to life while preparing for rapid transport to the appropriate receiving facility.
